Getting started at CrossPointe Wellness is quick and easy.
When you schedule an appointment, we’ll need a few pieces of information from you:
- Your first and last name
- The name of the primary insured person (if it’s someone other than you)
- The insurance company
- Your member ID number
- Group number (if applicable)
We’ll reach out to your insurance company to verify your coverage, any applicable co-pay, and any other information you’ll need.
CrossPointe Wellness is In-Network with the following insurance companies:
- Aetna BetterHealth of Kentucky
- Anthem / Blue Cross Blue Shield
- Humana Caresource
- Kentucky Medicaid
- Passport Health Plan
- United Healthcare
